Some commonly referred to examples of Preventative Care Physicals are:
Annual Exam; Well Man Visit; Well Woman/Pap; Annual Well Child Check; Kindergarten Physical; Sports Physical; Daycare Physical; DOT Physical; College Entrance Exams; and, Camp Physicals.
It is important to remember this is a well visit; therefore we will not be discussing new problems or conditions, changing medication dosage, or starting new medications. Any new issues will need to be addressed at another office visit.

Well Man
- physical
- CBC, CMP, TSH, Lipid and PSA (if over 50)
- prostate check
- review immunization status
- review need for screening colonoscopy, hemacult cards
- review current meds
- suggestions for healthy living
Well Woman
- physical
- Pap
- breast exam
- CBC, CMP, TSH, Lipid
- order screening dexascan/mammogram (if needed)
- review need for screening colonscopy, hemacult cards
- review immunization status
- renew current meds
- suggestions for healthy living
Annual Well Child Exam
- physical
- renew current medications
- review immunization status
- age appropriate suggestions for healthy and safe environments
Kindergarten Physicals, Sports Physicals, DOT Physicals, Daycare Physicals, Camp Physicals, and College Entrance Exams will be specific to requirements of form.
